Dear Alice,

I've heard that drinking coffee and grapefruit juice at around the same time can extend the effects of caffeine. Is this true? I know that grapefruits can increase the bioavailability of some drugs... what about caffeine? Is this at all dangerous?

Grapefruit juice is not only a popular breakfast beverage— it also becomes surprisingly active in your body. However, when it comes to extending the effects of caffeine, grapefruit juice likely isn’t going to give the boost you’re seeking. Since grapefruit juice can affect absorption of substances in the bloodstream (what you identified as bioavailability), health care providers often encourage patients to avoid grapefruit products while taking certain medications.  

Although grapefruit juice can affect how your body breaks down and absorbs substances, grapefruit juice and caffeine are processed differently. Therefore, for many people, sipping grapefruit juice with your morning coffee probably isn't going to have significant effects.  

How does grapefruit juice affect the body? 

Grapefruit juice interacts with the enzymes and proteins in your gut and liver. Gut and liver enzymes are responsible for breaking down many substances you consume, including certain medications, and grapefruit juice can slow down this process. As a result, health care providers might recommend patients avoid grapefruit juice while on certain medications. 

 How do grapefruit juice and caffeine interact? 

While the taste of coffee and grapefruit juice don’t seem to mix well, research shows they likely don’t interact in the body.  Caffeine and grapefruit juice are processed by different enzymes in the body. The enzymes that grapefruit juice are known to block aren't the primary enzymes responsible for breaking down caffeine. 

What else might you consider while drinking grapefruit juice? 

Other fruits can affect how substances break down and absorb  in the blood stream as well. This includes Seville oranges, pomelos, tangelos, and limes, so it's not just grapefruit you need to keep in mind. If your health care provider recommended you avoid grapefruit juice with certain medications, you may also want to ask about other forms of citrus juice.  

 For most healthy people, having grapefruit juice and coffee around the same time is unlikely to cause any significant caffeine boost or risk. That said, everyone's body is a little different. If you're on medications, have a health condition, or just want to be sure, the best move is always to check with your health care provider or pharmacist. They can give you personalized advice based on your unique situation and how much grapefruit you're consuming.
